Your Information May Have Been Leaked From a Recent Hack at Partnership HealthPlan of California

Levi & Korsinsky, LLP is investigating potential claims against Partnership HealthPlan of California. (“PHC”), a third-party administrator of MediCal programs, relating to a data breach that reportedly exposed over 800,000 clients’ personal information to an unauthorized party. Specifically, PHC determined that the information impacted includes name, Social Security Number, date of birth, Driver’s License number or Tribal ID number (if provided), medical record number, treatment, diagnosis, prescription and other medical information, health insurance information, member portal username, password, email address, and physical address.
