LifeVantage Corporation Information Request Form

Levi & Korsinsky, LLP announces the filing of a class action lawsuit in the USDC for the District of Utah on behalf of shareholders of LifeVantage Corporation (“LifeVantage” or the “Company”) (NASDAQGM: LFVN) who purchased shares between November 4, 2015 and September 13, 2016.

The complaint alleges that throughout the Class Period, Defendants made false and/or misleading statements and/or failed to disclose that: (1) LifeVantage lacked effective internal financial controls; (2) therefore, the Company had improperly accounted for sales in certain international markets, along with associated revenue and income tax accruals; and (3) consequently, LifeVantage’s public statements were materially false and misleading at all relevant times.

If you suffered a loss in LifeVantage you have until November 14, 2016 to request that the Court appoint you as lead plaintiff. Your ability to share in any recovery doesn’t require that you serve as a lead plaintiff.
